    ]]>     300-招待講演 sugawara 2008-05-19T09:05:16+09:00     「Socio-Orthodontics 研究会」設立趣意書 http://www.junji-sugawara.net/2008/01/12-175455.php   医療の対象になっているいかなる「病気」も、常に二つの異なったものの見方でとらえられています。すなわち、一方は、医療側の病理学的概念としての「疾病」(disease)という見方であり、他方は、患者あるいは人々の文化的概念としての「病い」(illness)という見方です。言葉を換えれば、前者は近代医学によって構築された「生物医学モデル」の概念であり、後者はそれぞれの地域、文化、社会の中で構築された「文化・社会的モデル」として人類の歴史とともに存続してきた概念と言えます。
      ところが、 disease 中心のものの見方をしてきた近代医療のパラダイムでは、昨今問題になっている「脳死」「臓器移植」「生殖医療」などに関しては、適切な対応が取りにくいことは明らかです。同様のことは、矯正歯科医療における「顔貌」や「審美」についても指摘できます。感染症のように disease と illness とが比較的接近している「病気」の場合には、医療の展開上あまり問題になることはありませんが、上述のように disease と illnessとの間に落差が存在する問題の場合には、その程度が大きければ大きいほど解決が困難になります。　本来、社会生活を営んでいる人間を対象とした医療は、自然科学だけではなく、人文科学および社会科学を加えた全体概念のもとに行われるべきであると考えられます。また、このようなアプローチなくしては、現代医療が抱えている問題を正しく認識し、その解決を図ることは困難であろうと思われます。
      「Socio-Ortodontics 研究会」は、このような視点に立ち、「文化・社会(society)と矯正歯科医療(orthodontics)との関わり」にスポットを当て、「顔貌」や「審美」などの具体的問題について学際的かつ実学的に研究し、社会および医療現場にフィードバックすることを目的としています。研究分野の違いを問わず、興味をお持ちの方々の参加を歓迎します。

    ]]>     500-Socio-Orthodontics研究会 (SOS) sugawara 2008-01-12T17:54:55+09:00     第15報　オランダでの外科矯正シンポジウム（BSSO） http://www.junji-sugawara.net/2007/12/07-143259.php 0711294-1jpg.jpg   2007年11月29日と30日にオランダのハーレムで開催された2007 Biennial Symposium Surgical Orthodonticsにおいて講演をしてきました。ハーレムと言えばニューヨークを思い浮かべるかも知れませんが、ニューヨークのハーレムはHarlemで、オランダのハーレムはHaarlemとスペルが少し違いますが、ニューオークのハーレムの語源になっているとのこと。ハーレムはアムステルダムせいの郊外にある閑静な町で、聖バフォ教会，グローテ・マルクト広場，テイラー美術館など，主な観光名所は徒歩で数分の所に集中しています。ハーレムのかつての繁栄ぶりは、空に向かってそびえ立つ聖バフォ教会から容易に想像されます。その迫力はかつて訪れたことのあるケルンの大聖堂に匹敵します。図１は私が泊まったホテルのカーテン越しに撮影した教会の写真です。教会内部に設置された巨大なパイプオルガンにも驚かされました（図２） 。

      さて、BSSOはオランダの外科矯正チームが中心になって隔年開催されています。今回は、私の長年の友人で、このチームに関わりのあるDr.Joop Drenbosの招きによるものです。「包括歯科」「CBCTを用いた診断」「治療計画の立案」「術前矯正」「非外科的治療」「顔面非対称の治療」「開咬を伴ったLong faceの治療」に関する７つのセッションあり、私はこのうちの３つのセッションでSurgery First、SASによる開咬の治療、成長期/成人期の下顎前突治療についてそれぞれ30分ずつ話をさせていただきました。
      私以外では、Dr. G William Arnett、Dr. Vincent Kokich、Prof. Ravi Nanda、Dr. James Mah、Dr. Monica Palmerなどのビッグネームが講演者として招待されていましたが、彼らとはもう何度か他の学会やシンポジウムでも顔なじみになっており、その分だけリラックスした気分で講演をすることができました。それぞれ興味ある内容でしたが、とりわけDr. Palmerの「外科矯正治療計画における失望（Disappointments in Orthognathic planning）」と題した講演が印象に残りました。顎変形症の外科矯正治療においては手術失敗もさることながら、治療計画を立てる段階での失敗が見逃せない事実として存在するという問題提起でしたが、私も同じ思いを抱いていただけにとても納得できる内容でした。彼女はベルリンで開業している矯正歯科医で、 外科矯正を含む包括歯科を中心に診療をしていますが、同時にプロの画家でもあります。顎変形症の治療では、かみ合わせだけではなく容貌も扱うため、正解が分からない中で治療ゴール設定を強いられることから、術者にはスキル以外に芸術的センスが要求されます。患者の失望の多くは容貌に向けられることから、彼女の問題提起はいかにも芸術家らしい“ものの見方”を感じさせるものでした。

    ]]>     100-きょうの矯正歯科 sugawara 2007-12-07T14:32:59+09:00     第14報　第7回インプラント矯正研究会セミナー http://www.junji-sugawara.net/2007/11/30-102842.php インプラント矯正とは、各種インプラントを歯を移動するための固定源に利用するという画期的な治療法を意味します。インプラント矯正を歯科臨床において適切に定着させるため、当時インプラント矯正を手掛けていた臨床医が中心になって、2000年にインプラント矯正研究会を設立しました。そして情報交換の場として年1回のセミナーを10年限定で開催することにしました。当時、矯正用固定源に応用されていた材料はサンキン社製のミニインプラント（K1）とミニプレート（SMAP）しか販売されていなかったことから、サンキン社に事務局を置いて第1〜5回セミナーを開催しました。その後、サンキン社以外の製品も販売されるようになったことから、昨年から事務局をカノミ矯正・小児歯科クリニック（姫路市）に移して第6回セミナーを開催しました。そして、今年から当院SAS矯正歯科センターに事務局を移し、残り4回のセミナーの企画運営に当たることになりました。

     

    ]]>     100-きょうの矯正歯科 sugawara 2007-11-30T10:28:42+09:00     第13報　チャイルドライフ支援ボランティア講座 http://www.junji-sugawara.net/2007/11/10-114655.php 10月28日に、私が関わっているNPO法人ワンダーポケット（http://www.w-p.jp/）主催による2007年度のボランティア講座が仙台厚生病院・海老名ホールで開催されました（図１）。本講座は「すべての子どもたちに命の輝きを」というワンダーポケットの理念に基づくNPO活動の一環で行われているものです。病気や障害を抱えた子どもへの支援だけではなく、その兄弟姉妹やご両親への支援をも視野に入れたボランティアを養成することが目的です。

    ]]>     100-きょうの矯正歯科 sugawara 2007-11-10T11:46:55+09:00     第12報　第89回アメリカ口腔外科学会での講演 http://www.junji-sugawara.net/2007/10/20-101107.php ５月以来、久々の外国講演でしたが、ハワイで開催された第89回アメリカ口腔外科学会に出席してきました。今回は、アメリカだけでなく、日本と韓国を交えた初めてのジョイントミーティングでしたので、日本からも多くの口腔外科医が参加しておりました。7-8日に東京でDr. PancherzによるHerbst applianceのセミナーに参加して、その足で成田からホノルルに発ちました。口腔外科学会ですから、私のような矯正歯科医の参加はほとんどない訳ですが、今回出席した理由は、ワシントン州立大学口腔外科のDr. Jessica Leeからの依頼で、二人で「Skeletal Anchorage System: Clinical Protocols for Surgeons」というテーマでミニレクチャーをしようというお誘いを受けたからです。美しい韓国系アメリカ人の女医さんなので断れませんでした（図１）。

    ]]>     100-きょうの矯正歯科 sugawara 2007-10-20T10:11:07+09:00     第11報：ある再治療症例の治療を終えて（２） http://www.junji-sugawara.net/2007/08/12-090349.php 再治療とは何かについては第３報（2007年2月23日）で詳しく述べましたので、知りたい方はそちらをご覧下さい。ここでは、最近治療を終えた再治療例を紹介します。このクライアントをMさんと呼ぶことにします。きっかけは顎関節症状でした。その治療を求めて近くの病院歯科を訪れたところ、かみ合わせの異常を指摘され。紹介されて私のところにやってきました。Mさん（22歳女性）のお話しによれば、小学生の頃に関東の某大学歯学部附属病院矯正歯科において反対咬合の本格的治療を受けた経験があるとのことでした。図１は、初診時に撮影したお口の写真ですが、前歯部開咬と反対咬合を呈していました。上下顎の第一小臼歯が４本なかったことから、マルチブラケット装置による治療がなされたことが想像できました。また、Ｘ線写真から上下顎切歯の歯根吸収が著しいことも分かりました。治療前の状態が知りたくてMさんにお願いしましたが、残念ながら、もう10年以上前のことなのでその大学病院に資料は保存されていないとのことでした。

    ]]>     200-略歴（CV） dunamis 2007-06-13T11:55:21+09:00     第10報　口腔成育シンポジウムでの講演 http://www.junji-sugawara.net/2007/06/10-083310.php 仙台口腔成育シンポジウム・ファイナルが６月２・３日に仙台弁護士会館において開催されました（図１）。主催は仙台を中心にした歯科医グループで、今年は５年間限定開催の最終年ということでファイナルと銘打っていました。口腔成育とは、子どものバイオ・サイコ・ソーシャルに関わる包括的歯科医療や支援を意味し、子どもの継続的なお口のケアを通じて心の発達（セルフケア確立や自立）にも寄与することを目的としています。そして自分たちの身体や心を大切にしようとするマインドが次世代に継承されることへの期待もその中に込められています。

    2007年の B.F. and Helen E. Dewel Award　（通称The Dewel Award）が私たちの論文に与えられることになったからです。The Dewel Awardとは、矯正歯科学分野のトップジャーナルであるAmerican Journal of Orthodontics and Dentofacial Orthopedics (AJO-DO)が、その前年に掲載された全論文の中から、最優秀論文が選ばれて贈られる賞で、AJO-DOの賞としては最も権威のある賞とされています（図１）。授賞対象になった私たちの論文とは以下のような論文でした。

    ]]>     100-きょうの矯正歯科 sugawara 2007-05-21T19:11:10+09:00     第８報：アメリカ矯正歯科学会(AAO) での招待講演 http://www.junji-sugawara.net/2007/05/20-184116.php AAOの年次大会は世界最大規模の学術大会で、参加者はアメリカ国内にとどまらず、世界各国から毎年約２万人の会員が集まります。今回で107回目の年次大会であることからしても、いかに伝統のある大会であるかが伺えます（図１）。私の年次大会での招待講演は、2000年シカゴ、2002年フィラデルフィア、2006年ラスベガスに続いて今度のシアトルで４回目ですが、その度に緊張を強いられています。講演後に聴衆による評価がなされ、その採点表が数ヶ月後に講演者に送られてくるからです。評価が低い場合は、以後、招待講演の機会が少なくなるという厳しさです。コンテンツが大事なのは当たり前のことですが、英語の発音が悪くて出席者に内容が伝わらないというのも減点の対象で、日本人だから発音は大目に見て欲しいという甘えは通らないようです。私を含め、日本人にとっては辛いことですが、世界の共通言語が英語になっている現在、避けることのできないハードルなのです。だから、若い日本の矯正歯科医には、英語力を付けるための継続的努力をしなさいと、いつも口が酸っぱくなるほど言っているのですが、きっと彼らには現実味もなく、危機感もない話に聞こえるのでしょうね。

    ]]>     100-きょうの矯正歯科 sugawara 2007-05-20T18:41:16+09:00     第７報：コネチカット大学での講義 http://www.junji-sugawara.net/2007/05/17-180743.php 5月15日にレンタカーを運転してボストン（マサチューセッツ州）からファーミングトン（コネチカット州）に向かいました。フリーウェイの91 Westから84 Southに乗っての約２時間のドライブでしたが、天候は快晴で、新緑が太陽に輝いて、とても清々しい気分で気持ちよく運転できました。そして、医学部と歯学部そしてそれぞれの附属病院や研修施設などが組み込まれたUniversity of Connecticut (UCONN) Health Center（図１）に無事到着しました。

    このシリーズは、今から８年前のもので、私が東北大学で助教授をしていた頃に河北新報に連載したものです。少し賞味期限を過ぎた部分もありますが、殆どはこれから矯正歯科治療を受けようと考えている方には、今でもお役に立てる内容ではないかと思います。驚いたのはこのシリーズの反響です。予想以上に多くの方が読まれていたことを知りました。中には、切り抜いてスクラップブックにしていた方もおりました。また、治療を諦めていた方が、矯正歯科治療は何歳になっても可能なのだということを知り、実際に治療を受けて長年のコンプレックスを解消した方もいらっしゃいました。賞味期限が過ぎた部分はいずれ折を見て改訂したいと思っています。

    ]]>     700-新時代の矯正歯科（河北新報連載） sugawara 2007-05-17T15:55:25+09:00     第６報：16th International Symposium での招待講演と・・・ http://www.junji-sugawara.net/2007/05/15-173800.php ボストン大学が主催するインプラントロジーのシンポジウムに招待され（図１）、５月１３日にサンフランシスコ経由でボストンに飛び、その日の夜１０時頃にホテルであるBoston Marriotte Long Warfに到着しました。その時、古い友人でYonsei大学（韓国）の学部長でもあるDr. Young Chel Parkとばったりと出会いました。彼と暫く雑談をした後に部屋に入りましたが、すでに外食するような時間ではなかったので、ルームサービスを頼むことにしました。注文したのは、ボストン名物のクラブケーキ（蟹肉を固めて焼いたもの）とクラムチャウダー（図２）。久しぶりだったことと、お腹がすいていたこともあってか、とても美味かった。
